- Though
4 months passed, the PIO has
not supplied any information.
So, I request the Commission to
hear my case and issue
the following directions
in order to give justice
to me.
Prayer
a. Direct PIO to supply
the information free of cost as per section 7(6) of the RTI
Act.
b. Impose
penalty on PIO for
denial of information.
c.
Award
me compensation amounting Rs. 1000.00 for causing
me unnecessary mental tension and
harassment for depositing such huge amount

in transferring RTI Application by the PIO under section 6 (3) of the RTI Act
Respected Sir
I beg to present herewith the
gross negligence on the part
of PIO for making
unnecessary inordinate delay in transferring RTI application under section 6(3)
of the RTI Act.
A. That, On
23.2.19., I had filed RTI Application to the PIO, office of Revenue and Disaster
Management, Govt. of Odisha seeking
information about Bench Mark value of land within jurisdiction of
Bhubaneswar Municipal Corporation.
B. On 7.3.19.,
the PIO received my
application and after
around 21 days, on 28.3.19, ( letter No. RDM-Regn-RTI-0008-2019) forwarded
my RTI Application to another Public authority i.e., office
of District Sub- Registrar, Khurda
to supply the information.
C. But
interestingly, on 10.7.19, I received
the copy of the
forwarding letter of PIO. I
got astonished how a letter
dated 28.3.19 reached
me on 10.5.19 after around
42 days.
Then, I verified the postal
stamp with date on the
letter and found that the letter
was posted on 7.5.19.
D. I got pretty sure that
the PIO has manipulated the
letter date and sent
it with back date.
So, I request the Commission to
hear my case and
conduct an inquiry into
details of manipulation of date of
forwarding letter dated 28.3.19
which reached at me on
10.5.19 and impose
penalty on PIO for
playing dirty politics to attend RTI applications properly and
award me compensation of Rs. 5,000.00 for causing
huge mental tension to me.

Dear
Sir
I beg to state that on 12.11.18., I had filed RTI Application to the PIO, Office of Chief District Medical Officer, Nayagargh seeking information about Implementation of
Biju Swasthya Kalyan Yojana . On 27.11.18,
the PIO sent a letter asking me to deposit Rs. 72.00 towards cost of information. I deposited
the amount and received the information. But I found that
the PIO has demanded some illegal
fees like Regd. Postal charge for
sending intimation letter . This is
gross violation of RTI Act
I request the commission
to inquire into the matter and direct
the PIO to return the
extra money received by him and
impose penalty on PIO for
demanding illegal fees under section 20 (1) of the RTI Act.

That, I had filed RTI application to PIO, office of Collector, Gajapati seeking information about Details of human causalities due to
Titili cyclone. On 28.1.19., the PIO forwarded RTI Application
to the PIO, office of District Child Protection Unit, Paralakhemundi to
supply the information.
B.
On 23.2.19, the PIO, DCPU supplied some information but denied the following
information, as it is not available.
· information
about details of human causalities (
death ) due to Titili cyclone in
the district with name, village and
block.
· Details of fund
sanctioned for relief and rehabilitation
of the cyclone-affected and fund
utilized under different head till yet.
C.
I am under impression that the PIO with malafide intention just
forwarded to the PIO, DCPU without
supplying the information which are already available with him.
D. So,
I request the Commission to hear
my case and direct the PIO to supply the
information free of cost and impose penalty on PIO under section 20 (1)
of the RTI Act .

25.10.18., I had filed RTI application to the PIO, office of Housing and Yrban
development, Govt. of Odisha Bhubaneswar
seeking information about Details
of AHARA Yojana. After 66
days of receipt of
application, on 11.1.19, the PIO
transferred my RTI Application to
the PIO of all Collectors-cum-
Presidents , District Aahaar Society (DAS), Commissioner, BMC-cum-President,
CAS, Bhubaneswar . But the PIO of
Collectors of four districts like
Bhadrak district, Subarnapur, Keonjhar and Kondhamal have
provided information. But the PIOs of Collectors of other 26 Districts and Commissioner , BMC have not
provided information, though 4
months passed since RTI Application
forwarded by the PIO. The PIO
could have supplied the information without
forwarding the same after 66 days
and asking all offices of Collectors
to supply the information. The PIO
has grossly neglected
to deal RTI Application and party to denial of information. It is not
practically possible to file
27 no. of first appeals to get the
information.
I request the Commission to hear the case and
constitute an inquiry into it and impose
penalty on PIO for
denial of information.

B. On 28.3.19., the PIO
sent Form-C rejecting
my application citing
the ground “ Your Identity is not
satisfactory”.
C. While submitting RTI
Application, I have submitted my Adhar
Card as Identity proof following Odisha RTI Rules, 2005, though it
is illegal under section 6 of RTI Act.
D. Despite submission of Adhar Card, the PIO has rejected
my RTI Application not
justifying how my identity
is not satisfactory.
So, I
request the Commission to
issue the following
directions in order to
give justice to me.

a. Direct PIO to receive
my RTI Application and provide me
information within 3 days .
b. Impose
penalty on PIO, for arbitrarily interpreting
the law and rejecting RTI Application.
c. Award me
compensation amounting Rs.

said information was crucial for me during
that time.

I beg to present herewith the
gross negligence on the part
of PIO for demanding illegal fees towards cost
of information.
A. That, On
25.2.19., I had filed RTI Application to the PIO, office of School and Mass Education
Bhubaneswar
seeking information about position of post
of Urdu teachers in different
Govt. schools in Odisha
B. The
PIO transferred my RTI
Application to all BEOs
to provide the information.
C.
On 6.5.2019, the PIO, office of BEO,
Bhapur, Nayagarh wrote a letter
to deposit Rs. 70.00
towards Regd. Post and speed post charge without giving any details of cost of
information. I also got astonished how the PIO
demanded fees for both
regd. Post and speed post charge .
D. I
am under impression that the fees demanded by the PIO
is illegal and outrageous.
So, I request the Commission to
hear my case and
conduct an inquiry into details of
it and impose penalty on PIO
for sending such type of
confusing and demanding illegal fess. at me on 10.5.19 and
impose penalty on PIO
for playing dirty
politics to attend RTI
applications properly and award me compensation of Rs. 5,000.00
for causing huge mental tension to me.

B. On5.2.19, the PIO forwarded

C. On 7.2.19, Addl.PCCF forwarded
the RTI Application to the PIO, office of DFO, Chandaka to provide information.
D. Though 4 months passed, the PIO has not supplied any
information.
So, I request the Commission to hear my case
and issue the following
directions in order to
give justice to me.
Prayer
a. Direct PIO to supply
the information free of cost as per section 7(6) of the RTI Act.
b. Impose
penalty on PIO for
denial of information.
c. Award me

sent a letter asking to deposit
Rs. 100.00 towards cost of information in which the PIO has made charge of Rs. 44.00

deposited fees amounting Rs. 3600.00 through Money Order.
C. Though 25 days passed, the
PIO has not supplied any information.
So, I request the Commission to hear my case

resources , Govt. of Odisha
seeking information about Implementation of various programme. It was eventually
transferred to the PIO, office of Executive Engineer, Minor Irrigation
Project, Khorda. On 29.11.18., the PIO
sent a letter asking to deposit

I deposited the amount and collected
the information. But PIO has made charge of Rs. 22.00
towards postal charge of

PIOs of CDPOs of Koraput district
Respected Sir
I beg to present herewith the
gross negligence on the part
of PIOs of all Child Development Project Officers of Koraput district for
non-supply of information under RTI Act.
A.That, 0n 6.9.18., I filed RTI Application to the PIO , office of Women and Child Development, Govt. of
Odisha seeking information about Construction of Anganwadi
centres in different districts under various schemes.
B. On 29.9.18, the PIO forwarded my RTI Application to the PIO, office of Collector, Koraput to supply
information. The PIO, office of
DSWO, Koraput forwarded the same to all CDPOs
to supply the information.
C. Though
more than 5 months passed, no CDPOs
supplied the information.
D. I am under impression that both DSWO and all CDPOs with malafide intention have denied
to provide this information.
So, I request the Commission to
hear my case and issue
the following directions
in order to give justice
to me.
Prayer
a. Direct PIO of DSWO, Koraput
to collect all information from
CDPOs and supply the same free
of cost as per
section 7 (6) of the RTI Act.
b. Impose
penalty on PIOs who have neglected in supply
of information.
c. Award me
compensation amounting Rs.
1000.00 for causing me unnecessary mental tension and harassment for denying information.
